SCASL is looking for a passionate person who wants to make a difference in the lives of individuals living with sickle cell disease and their families.
Role Overview: As a CHW, you’ll play a vital role in providing care coordination for individuals with sickle cell disease, offering social support, navigating health systems, and facilitating access to resources. You’ll serve as a beacon of support and guidance, helping bridge medical services and empowering patients to enhance their healthcare engagement and outcomes.
Duties and Responsibilities:
- Assist in developing care plans for sickle cell clients.
- Evaluate medical, insurance, and social needs of clients.
- Coordinate educational seminars and workshops within designated parishes.
- Conduct outreach at community health fairs and events.
- Attend Sickle Cell Clinics in Baton Rouge and New Orleans monthly.
- Assist in establishing support group activities.
Qualifications, Skills, Knowledge:
- Proficiency in Microsoft Office Programs (Word, Outlook, Publisher, Access, Excel, QuickBooks).
- Excellent communication skills (written and verbal).
- Exceptional organizational abilities.
- Ability to work well under pressure and multitask effectively.
- Detail-oriented with a proactive approach.
- Strong facilitation, organization, and presentation skills.
- Punctuality is a must.
- Passionate.
Travel:
- Local travel within parish service areas.
- Occasional out-of-town travel for training or conferences.
- Some weekend availability may be required.
Schedule:
- Monday through Friday: 9:00 a.m. – 4:30 p.m.
- Some weekends as needed.
